Download, print and fill out the following five forms:  Application and Release, Medical History and Physician's Statement, Liability Release, Emergency Medical Authorization, and Confidentiality Policy and Photo Release. Or contact Equul Access, Inc. to request paperwork.

Return completed forms to place the potential participant on the Waiting List. Remember, the Medical History and Physician's Statement must be completed, signed and dated by the participant's doctor in order to be valid.

Attend the scheduled Evaluation.

Participants are placed into the program based upon information obtained from review of the forms, the evaluation, scheduling needs/availability, etc.
