THERAPEUTIC HORSEMANSHIP

Our focus is always ABILITIES. Find out for yourself how Therapeutic Equine Assisted Activities can benefit you, your friend, your loved one or your client.

Participants are placed into the program based upon information obtained from review of forms, an evaluation, and scheduling needs/availability.

To participate, please:

Submit the following five forms (via mail or electronically):

• Application and Release (pdf)
• Returning participants can fill out the Participant Update(pdf) instead
• Medical History and Physician's Statement (pdf) (signed and dated by the participant's doctor)
• Liability Release (pdf)
• Emergency Medical Authorization (pdf)
• Confidentiality Policy and Photo Release (pdf)

Upon receipt, forms will be reviewed for eligibility assessment. We will schedule an evaluation as appropriate.The evaluation is 30-45 minutes and may involve getting on a horse.

After the evaluation, we will discuss scheduling the participant into the program as appropriate.
